Documentation Required for Loan Modification

To start the evaluation process, First Fed will need the following:

  • Completed Loan Modification Package that includes the following forms (click here for forms):
    -Personal financial statement
    -Letter agreement
    -Request for Transcript of Tax Return IRS Form 4506-T
  • Copies of your most recent monthly bank statements for all accounts (checking, savings, 401(k), CDs, money markets, etc.)
  • Copy of promissory note if a 2nd Trust Deed (home equity loan or home equity line of credit) exists on your current property
  • For salaried employees:
    -Copy of your W-2 for the most recent tax year
    -Copy of your current paystub
  • For self-employed earners (if applicable):
    -Signed and dated copy of your IRS Form 1040 for the most recent tax year including all schedules
    -Signed and dated copy of your Corporate Tax Returns for the most recent tax year including all schedules, if applicable
    -Signed and dated copy of year-to-date Profit & Loss Statement, most recent month-end and quarter-end Balance Sheet and:
    -Articles of Incorporation, Oganization and Corporate By-Laws or Operating Agreement
